LOCATION: ELEMENTARY SCHOOL

K4 students must be four and Kindergarten students must be

five prior to the first day of school, August 31, 2021

You may pick up a registration packet or you can print the forms listed at the bottom of this page and complete prior to your appointment

PLEASE BRING:

• BIRTH CERTIFICATE OF STUDENT
• IMMUNIZATION RECORD OF STUDENT
• COURT ORDER/CUSTODY AGREEMENT (IF APPLICABLE)
• PROOF OF RESIDENCY
• PARENT/GUARDIAN PHOTO ID WITH CURRENT ADDRESS

(It is also strongly recommended that you bring a copy of a physical and dental exam completed within the last 6 months)
