2025-2026 STUDENT SPORTS PHYSICALS/ PAPERWORK UPDATE
Dear Parent/ Guardian of a Union City Area School District Student,
The PIAA requires ALL sport athletes in grades 7th through 12th to have a physical and PIAA forms completed and submitted before the first day of mandatory practice.
PIAA sports physicals begin and expire with the school year (May1st- April 30th). Any physical dated before May 1st, 2025 will not be accepted for the upcoming school year.
ALL PHYSICAL PAPERWORK WILL BE DONE ONLINE at ucsd2.atsusers.com
New user: Directions have been listed below called: ATS Directions
Returning User (has played a FALL and/or WINTER sport OR has attended a sports physical day): Directions have also been linked below called: ATS section 7 Recertification Directions
Returning User (has NOT played a FALL and/or WINTER sport OR has NOT attended a sports physical day): Directions have also been linked below called: ATS sections 1-5 Directions also a PIAA section 6 physical must be completed by a Doctor.
ALL PHYSICAL PAPERWORK MUST BE COMPLETED ONLINE PRIOR TO THE FIRST DAY OF MANDATORY PRACTICE.
(JH Volleyball: February 17th,VarsityTrack + Field, Softball and Baseball March 2nd, and JH Track March 30th)
No paper copies will be accepted, unless you go to your primary care physician and they fill out the PIAA section 6.
