2026-2027 STUDENT SPORTS PHYSICALS/ PAPERWORK UPDATE
Dear Parent/ Guardian of a Union City Area School District Student,
All athletes that are interested in playing a FALL, WINTER and or SPRING sport are recommended to attend the physicals on Tuesday May 26th for a cost of $10.
The PIAA requires fall sports athletes from 7th through 12th grade must have a physical completed and submitted before the respective mandatory practice.
PIAA sports physicals begin and expire with the school year (May 1st- April 30th). Any physical dated before May 1st, 2026 will not be accepted for the upcoming school year.

New user: please read the registration directions on the Union City web page under the athletic/ athletic announcement tab.
Returning User: If you have forgotten your athlete ID and password please email Alexis Barron our Athletic Trainer (abarron@ucasd.org), DO NOT make a new account.
ALL PHYSICAL PAPERWORK MUST BE COMPLETED BY 8pm MAY 25th.
**Your student will receive a time via email for their “appointment time” for physical day once their online paperwork is complete**
If online paperwork is not completed or $10 payment is not present your student athlete will not be receiving a physical and will be turned away. Checks made payable to: Union City Area School District
Every year you will need to complete the online PIAA sections 1-5 physical paperwork online.
No paper copies will be accepted, except for the PIAA section 6 physical paper filled out by your doctor.
